<sec><title>Background</title>
<p>Throughout history, peoples live through vicissitudes from situations of security, peace, recovery to situations of war and the vicissitudes of nature and its cruelty over the diversity, intensity. The current study aimed to evaluate the self-esteem (SE) of patients who are suffering from amputees at Kut city, in Middle Iraq.</p>
</sec>
<sec><title>Methods</title>
<p>The descriptive crosssectional design is selected to determine body image (BI) and self-esteem (SE) among the amputated patients and to identify if the body image has been associated with self-esteem. By nonprobability purposive sample composed of 300 subjects from both gender male and female with a different age. The pilot study was applied to establish the questionnaire's dependability before it was presented to experts to test validation. The data were gathered through the use of interviews, and it was then evaluated utilizing a descriptive statistics &#x0026; inferential statistical data analysis strategy.</p>
</sec>
<sec><title>Results</title>
<p>The study results shows that the more participants is male patients and their aged between (50-59) years old. Majority amputation site caused by diseases have recorded was the lower limbs also the war for more than 10 years. The most of amputated patients have a negative responses towards body image of them . Additionally, the majority of responses from patients who had limbs amputated show that they never felt confident in themselves (57%) Self-esteem and body image had a positive correlation (r=0.574; p=0.000).</p>
</sec>
<sec><title>Conclusions</title>
<p>Amputated patients regard themselves adversely, which leads to low SE, while a negative BI affects self-esteem. To increase the possibility of early intervention, hospitals need to have specialists in mental health and psychological therapy. in these instances and to guide amputation cases to reduce psychological issues.</p>

<title>INTRODUCTION</title>
<p>Amputation in and of itself, from the scientific and life aspect, is considered a disability, and amputation is considered a war injury and a disability in which the affected individual goes through multiple stages that change the system of feeling security and peace for the amputee (<xref ref-type="bibr" rid="ref1">1</xref>). Amputation rates vary by country, ranging from 1.2 to 4.4 per 10,000 inhabitants (<xref ref-type="bibr" rid="ref2">2</xref>) (<xref ref-type="bibr" rid="ref3">3</xref>), The bulk (up to 90%) of cases affect the lower limbs. By 2050, these figures are predicted to more than treble (<xref ref-type="bibr" rid="ref4">4</xref>). Amputation causes a number of restrictions on one's ability to engage in social, professional, and recreational activities. (<xref ref-type="bibr" rid="ref5">5</xref>). Amputation is frequently an unavoidable treatment in the advanced stages of various diseases, including accidents, war, diseases as diabetes mellitus (DM), peripheral artery occlusive disease, cancer disorders, trauma, or infection, and has a significant impact on a patient&#x2019;s life (<xref ref-type="bibr" rid="ref6">6</xref>).</p>
<p>The image of the body is a mental image that the individual forms of his body, whether in its external appearance or in its internal components and its various organs and its ability to employ these organs and prove their efficiency and the accompanying feelings or positive or negative attitudes about those mental images of the body (<xref ref-type="bibr" rid="ref7">7</xref>). Quality of life is defined by the World Health Organization as &#x201D;the individual&#x2019;s assessment of their place in the conditions of the culture and values in which they live, as well as in connection to their objectives, expectations, principles, and interests.&#x201D; Demographic and health characteristics have been shown to influence quality of life and self-esteem in many domains of human existence, including physical, mental, spiritual, and social dimensions (<xref ref-type="bibr" rid="ref8">8</xref>) (<xref ref-type="bibr" rid="ref9">9</xref>).</p>
<p>BI is a dynamic and complex process that describes a person&#x2019;s individual impression of his or her own body. Internal elements such as age, gender, and physical condition influence this perception, as do external factors such as social or environmental circumstances (<xref ref-type="bibr" rid="ref10">10</xref>). SE is a favorable or negative attitude toward oneself: an overall evaluation of one&#x2019;s own worth or value. SE includes beliefs and emotions including triumph, misery, pride, and humiliation (<xref ref-type="bibr" rid="ref11">11</xref>). Therefore, thus present study aimed to investigate the body image (BI) and self esteem (SE) among patients with amputation at Kut City middle Iraq.</p></sec>
<sec id="sec1-2">
<title>MATERIALS AND METHODS</title>
<sec id="sec2-1">
<title>Study Design</title>
<p>A cross-sectional study design was chosen to achieve the aims of the current study. The descriptive correlational study design is done through the limit includes the following:</p>

<list-item><p>The study was limited to The Prosthetics Center in Kut, Iraq</p></list-item>

<sec id="sec2-2">
<title>Instruments</title>
<p>The tools used to conduct the study focus on the following:</p></sec>
<sec id="sec2-3">
<title>Socio-demographic characteristics</title>
<p>This contain age, gender, marital status, son, site of amputation, causes of amputation and duration.</p></sec>
<sec id="sec2-4">
<title>Body Image Questionnaire</title>
<p>A body image questionnaire which adopted and developed by Bruchon-Schweitzer, the scale measured on 7 point include (1 for very strongly disagree, 2 for strongly disagree, 3 for mildly disagree, 4 for neutral, 5 for mildly agree, 6 for strongly agree and 7 for strongly agree) (<xref ref-type="bibr" rid="ref12">12</xref>).</p></sec>
<sec id="sec2-5">
<title>Self Esteem Questionnaire</title>
<p>Based on a thorough examination of the literature, expert advice, and their own work with amputee patients, the authors developed a particular tool.</p>
<p>and a preliminary investigation on a group of 30 patients which approximately 10% from original sample who were asked open&#x2013;ended questions consist of ''20-items measured on 3-point as Never=1, Sometime=2, Always=3''.</p></sec>
<sec id="sec2-6">
<title>Validity</title>
<p>A panel of ten arbitrators, which included authorities in nursing science and psychology, was accorded validity. Arbitrators were requested to express their views and recommendations on the language appropriateness, relationship to the study variable's assigned dimension, as well as suitability for the study population of each component of the study questionnaire.</p></sec>
<sec id="sec2-7">
<title>Reliability</title>
<p>Data from amputation cases were used to evaluate the reliability of the questionnaire, and 30 patients who made up about 10% of the research group but weren't included in the original sample were given the test. Below is Cronbach's alpha.</p>
<p>Over time, the research population's confidence in the scale grows. It was stable during the initial test, and after two weeks, it also attained increased stability.</p></sec>
<sec id="sec2-8">
<title>Statistical Analysis Approach</title>
<p>The SPSS software application version 20.0 was used for doing statistical analysis (SPSS). The information was evenly spread. Correlation coefficients were used to test variations in variables. For continuous variables, descriptive data is reported as frequencies and percentage. Statistical significance was defined as a p <italic>≤</italic>0.05.</p></sec>

<sec id="sec1-3">
<title>RESULTS</title>
<sec id="sec2-9">
<title>Sample characteristics</title>
<p>Among 300 cases, the age group 50-59 were highest and 4-49 were lowest, the amputation was common in male (70%) than the female (30%), most of respondent were marred (75.3%) and the small ratio were widower, more than half were had son (60%), most of amputation cases in the lower limb (81%) and less cases in the joint (3.3%), the major causes of amputation were chronic diseases (45.7%) followed by war (36%), for more than 10 years (<xref ref-type="table" rid="table001">Table 1</xref>)</p></sec>
<sec id="sec2-10">
<title>Body Image and Self Esteem</title>
<p>The results revealed that (78%) of the amputees showed poor perception of body image and that (57%) of them showed low self-esteem (Figure 1; <xref ref-type="table" rid="table002">Table 2</xref>).</p></sec>
<sec id="sec2-11">
<title>Body Image vs. Self Esteem</title>
<p>Findings reveal that there was positive significant relationship between BI and SE (r = 0.754; p=0.000) (<xref ref-type="table" rid="table003">Table 3</xref>).</p></sec>
</sec>
<sec id="sec1-4">
<title>DISCUSSION</title>
<p>According to our findings, most of the study participants were men, aged (50-59) years. The lower limbs of patients have recorded the highest amputation site and caused by the wars for more than ten years.</p>
<p>The current results come because the male was share in war and in our society, male considered all the repossibilities of work so he is at risk of amputation, while females are have amputation due to congenital conditions or other defects.</p>
<p>The war records the highest percentage among the causes of amputation; The majority of the participants were elderly because the most of the wars had already ended. The investigation that was applied on 42 patients who had lower limb amputations corroborated our findings. (<xref ref-type="bibr" rid="ref13">13</xref>).</p>
<p>The results revealed that (78%) of the amputees showed poor perception of body image and that (57%) of them showed low self-esteem (<xref ref-type="table" rid="table002">table 2</xref>). These findings are due to the fact that amputation is a permanent surgery that leaves no residual body parts, which raises questions in cases of amputation due to uncertainty over the appropriateness of compensation for the amputee's lost body parts.</p>
<p>Many studies confirmed that low self-concept of the individuals with disabilities in the upper and lower peripheral exhibit heightened problems, particularly in terms of worries, feelings of guilt, and self-concept. (<xref ref-type="bibr" rid="ref14">14</xref>).</p>
<p>The study results illustrate that the body image may affect self-esteem and correlated (positive) (r=0.574; p=0.000).</p>

<p>The investigator attributes the existence of this association to the fact that one's perception of one's body has an effect on one's sense of self-worth. In other words, the more negatively one perceives one's body, the lower one's self-esteem will be, and vice versa. This relationship is expected given that one's perception of one's body is linked to future optimism and aspirations.</p>
<p>Given that amputation is a permanent procedure that prevents the amputated portion from growing back, amputees may feel inferior because there is no assurance that the compensation for the amputated part is sufficient. (<xref ref-type="bibr" rid="ref15">15</xref>).</p>
<p>The conclusions lead to the case of amputation before the person is affected by the understanding of the individual to self-acceptance, and they assisted in maintaining this opinion as a result of the amputation. As a result, the individual in the case of monitoring and evaluating themself is known as ''contemplating'' the state of the individual that develops low self-concept of the individual on the case anxiety that resulted in the amputation of the restraint.</p>
<p>This outcomes are agree with a study that done in Australia to assessed body image and self-esteem. The study found that amputation of the lower limbs had a considerable impact on the patient&#x2019;s body image. Self-esteem appears unaffected by the lower limb amputation, on the other hand. However, a feeling of phantom pain has a significant impact on one's self-esteem. Medical professionals thought that this condition was connected to a psychological issue. (<xref ref-type="bibr" rid="ref16">16</xref>).</p>
<p>Past Studies have established that there is a relationship between BI and SE and that there is a clear effect of BI on anxiety among individuals in general and specially in an amputee patients, as they confirmed that the image of the body has a clear effect on influential what the future is, making the person more anxious and isolated, because amputation is considered a disability phenomenon, It causes the person's body image to be distorted, and the new body image makes the person more erratic than his healthy peers, resulting in emotional disorders and social isolation. (<xref ref-type="bibr" rid="ref17">17</xref>).</p>
<p>It was behind BI estimation, the self-identity changes after the appearance of an amputation of the lower body limbs that affect performance, there by weakening all patients in different levels of education, leading to undesirable expectations about them in the future (<xref ref-type="bibr" rid="ref18">18</xref>).</p>
<p>The Limb Phantom phenomenon, which is also known as the anxiety-inducing belief that the amputated limb is still there and still feels, is supported by this. This phenomena directly harms the individual and strengthens anxiety in this area, playing a definite role. (<xref ref-type="bibr" rid="ref19">19</xref>) (<xref ref-type="bibr" rid="ref20">20</xref>).</p>
<sec id="sec2-12">
<title>Study limitation</title>
<p>SE and BI are difficult issues that are influenced by a variety of circumstances. The questionnaire was created based on psychological factors and may not accurately reflect the relationship&#x2019;s true complexity. As a result, data interpretation should be approached with caution.</p></sec>
</sec>
<sec id="sec1-5">
<title>CONCLUSIONS</title>
<p>The current analysis reflects that amputated patients were Negative self-perception results in low self-esteem. To increase the possibility of early intervention, hospitals need to have specialists in mental health and psychological therapy. in these instances and to guide amputation cases to reduce psychological issues. Additionally, offer programs to help amputation patients in order to lessen the psychological issues associated with amputation.</p>
